Health equity learning cohort

Mobilizing communities to improve regional health and well-being.

UPDATE

May 31, 2024  -  We are thrilled to announce the conclusion of our RFA process for the Health Equity Learning Cohort. This initiative aims to foster collaboration among organizations and communities in

central western Washington to create a shared vision for regional health and well-being.

We extend our heartfelt thanks to everyone who applied.

What does health equity mean to CHOICE?

At its core, health equity means that everyone, regardless of their background or circumstances, has what they need when they need it to live a healthy, thriving life. It's about ensuring fairness, justice, and equal access to healthcare, social services, and opportunities for all individuals and communities.

Health equity recognizes and addresses disparities in health outcomes, acknowledging that systemic injustices and social determinants of health play a significant role in shaping people's well-being; however, understanding and addressing disparities is not the full story.

Eliminating disparities is necessary, and it is also not sufficient –

we are focused on ensuring everyone thrives.

Meet the awardees

After careful consideration, we are excited to introduce the ten organizations selected to join the Cohort:

  • Best Dental Help

  • Child and Adolescent Clinic

  • Enterprise for Equity

  • Gather Church

  • Grays Harbor Public Health

  • Lewis County Autism Coalition

  • Love Overwhelming

  • South Sound YMCA

  • Wild Grief

  • Willapa Behavioral Health

What's next?

Between June 1 – December 31, these organizations will embark on a journey of listening, learning, and connecting with local leadership and communities. Through monthly meetings and focus area workgroups, the Cohort will explore the Vital Conditions of Health and Well-being framework to identify and connect cross-sector efforts, ultimately leading to recommendations for a regional health equity improvement plan.

Funding and commitment

Each organization will receive $35,000 to support their participation in the Cohort activities. Participants will commit to attending monthly meetings, which will include both in-person and remote sessions, and engage in at least one focus area workgroup. We look forward to the valuable insights and contributions these organizations will bring to our collective efforts in improving health equity.

Looking ahead

Stay tuned for updates on the progress and achievements of the Health Equity Learning Cohort. We are eager to see the innovative solutions and collaborative efforts that will emerge from this initiative, driving us towards a future where everyone in our region has the opportunity to live a healthy, thriving life.
